How Much Is Dangerous

Toxic Compound

Acetaminophen (paracetamol) itself is only part of the story — the real danger comes from how cats process it. Cats have very low activity of the liver enzyme glucuronyl transferase, the pathway most species rely on to safely break the drug down into harmless metabolites for excretion. With that pathway largely unavailable, a much larger proportion of any given dose gets shunted through a secondary metabolic route that produces a toxic byproduct called NAPQI (N-acetyl-p-benzoquinone imine). NAPQI directly damages red blood cells — causing methemoglobinemia, a state where hemoglobin is chemically altered so it can no longer carry oxygen normally, plus Heinz body formation and hemolytic anemia — and separately causes direct liver cell damage.
